INSTRUCTIONS FOR FILLING THIS FORM

1.  Either TYPE or WRITE the entries on the form neatly.

2. All items marked with an asterisk (*) require you to submit an attested copy of the document. PLEASE DO NOT SEND  ORIGINALS. Documents for which attested copies are required are given in the checklist. Please tick off the items in this checklist that you are attaching to this form.

3. The application processing fee (including Entrance Exam fee) is Rs. 1500/- (Rupees one thousand five hundred only ) (non-refundable) which must be deposited either in cash or online through Demand Draft payable to “Swami Vivekanand Subharti University” payable at Meerut only. Those who have downloaded the application form through website and wants to submitted offline will have to attach a Demand Draft of Rs.1500/- (Rupees one thousand five hundred only).

Academic Record/Work Experience

(A) - GATE, CEED, CSIR, UGC, other fellowship exam / M. Phil. details

(B) - Qualifying PG Degree (*) Those appeared in final exam may also apply. *

(C) - Other degrees (*) obtained after 10+2 & graduation, do not include the degree listed in (B).

(D) - Working Experience.

Note: Relaxation of 5% marks in eligibility criteria for SC/ST/OBC (non-creamy layer)/ Differently abled categories candidates will be given (if relevant certificate attached).
